HOSPICE SERVICES ENCOMPASS A RANGE OF CARE AND SUPPORT PROVIDED TO INDIVIDUALS WITH LIFE LIMITING ILLNESSES AND THEIR FAMILIES. THESE SERVICES ARE DESIGNED TO ENHANCE THE QUALITY OF LIFE, PROVIDE COMFORT AND ENSURE THAT PATIENTS AND THEIR LOVED ONES RECEIVE COMPREHENSIVE CARE. HERE ARE SOME KEY COMPONENTS OF HOSPICE SERVICES:

1.PAIN AND SYMPTOM MANAGEMENT

Hospice provides expert pain management and symptom control to alleviate physical discomfort and the patient's overall well-being.

2.SKILLED NURSING CARE

Registered nurses and nurse practitioners are available to monitor the patient's condition, manage discomfort, administer medications. In addition, they will also provide medical guidance, emotional support and self-care techniques.

3.PHYSICIAN OVERSIGHT

A hospice physician or medical director works closely with the patient's primary care physician to develop and adjust the care plan as needed.

6.PERSONAL CARE ASSISTANCE

Certified nursing assistants (CHHAs) assist with activities of daily living (ADLs) such as bathing, dressing and grooming to ensure the patient's comfort and dignity. Podiatry services assist in issues with feet, ankles and lower leg. They evaluate and treat lower leg issues. Wound Care specialists offer treatment for deep wounds that require special care including wound cleansing, irrigation and debridement.

4.PSYCHOSOCIAL SUPPORT

Trained social workers offer psychosocial support to patients and their families, counsel around issues of death, dying and grief, provide financial resource referrals and to assess emotional factors to help them cope with the emotional challenges of terminal illness.

5.SPIRITUAL CARE

Chaplains or spiritual counselors conduct and provide prayer worship, spiritual support, guidance and comfort based on the patient's spiritual beliefs and preferences.

7.BEREAVEMENT SUPPORT

Hospice provides grief counseling and support services for families before, during and after the patient's passing.

8.MEDICATION AND MEDICAL EQUIPMENT

Hospice provides medications, medical supplies and (DME) durable medical equipment related to terminalillness. Medications, medical supplies and DME are convenientlycand promptly delivered to the patient’s home. Supplies may include adult diapers, pull ups, gloves, incontinent supplies, wet wipes, bandages, chux and more. DME may include hospital bed, low air mattress, shower chair, walker, wheelchair, oxygen concentrator, oxygen tank an dmore.

9.VOLUNTEER SUPPORT

Trained volunteers offer companionship, respite care for family caregivers and assistance with various tasks. Some examples are helping prepare meals, running errands and picking up medications.

10.FAMILY EDUCATION

Hospice staff educates families on how to care for their loved one and provides guidance on what to expect during the end-of-life process.

11. 24/7AVAILABILITY

Although the office hours are Monday through Friday, 9AM-5PM, Hospice team on-call staff members are available around the clock to address any urgent needs or concerns.

12. COORDINATION OF CARE

Hospice professionals coordinate care across different healthcare providers and services to ensure a seamless and integrated approach to patientcare.

13. COMFORT CARE AT HOME

Whenever possible, hospice care is provided in the patient's home, allowing them to remain in a familiar and comfortable environment.

14. SUPPORTIVE THERAPIES

Services may include music therapy, art therapy, massage therapy and other complementary therapies to enhance the patient's comfort and well-being.
